CONCIERGE COMPANION
The concierge companion is responsible for making sure the client is taken to activities
as appropriate and is receiving the right care, offering companionship, reporting any
problems to you, the Concierge Nurse business owner.
The Care Manager will supervise the concierge companion and visit the client once a
month or as needed and contracted. ( this can be done remotely via telephone if
needed).
This service is offered whether the client lives at home, in a skilled nursing facility or in a
residential care facility. You will have to talk to the care staff or someone from the facility.
If the client lives in assisted living, confer with facility staff or a social worker or with the
care staff from the private duty home care agency employed by the client.
If the client lives in a residential care facility, confer with the care staff or residential care
staff member. If the client lives in a skilled nursing facility, discuss the care plan with the
charge nurse or staff nurse and review the client’s medical records.
You will be Co-supervising, along with the private duty home care agency, the care staff
who will follow the care plan to monitor the client’s care, take the client to activities, and
make sure the client’s needs are being met.
• Consulting with physicians, discharge planner, public agencies, professional third
parties, conservators, and family members as needed to confer about and update client
status.
• Accompanying the client to physician’s appointments.
• Conducting family meetings if communication among the family is affecting client care.
• Consulting with physicians, facility staff, family members, conservators/guardians, and
others as needed to confer about and update client status.
• Remaining on call for contracted hours in case of emergencies, questions,
optional.
• Writing reports as needed, making telephone calls, or sending e-mails as needed to
keep the client representatives informed about the client’s status and level of care.
• If activities need to be changed because the client is not responding well, suggest new
activities to the concierge companion, and delete old activities.
• Update the care plan.
• If a major problem, such as improper care by the facility, cannot be solved, contact the
client’s representatives.
